Alanté Health is seeking a dedicated RN Manager that will manage and oversee the Chronic Condition Management (CCM) program, which provides essential support to patients with conditions such as diabetes, heart failure, COPD, hypertension, and CKD. The RN Manager holds direct leadership responsibility for a multidisciplinary team of registered nurses, case managers, and care coordinators, ensuring the delivery of evidence-based, patient-centered care while maintaining compliance with all applicable healthcare standards.

Responsibilities

  • Lead the Care Team: Directly supervise, mentor, evaluate, and train the multidisciplinary team delivering Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services.
  • Drive Quality Outcomes: Implement evidence-based practices to improve patient outcomes, reduce readmissions, and enhance patient experience.
  • Ensure Compliance: Guarantee adherence to all regulatory and quality standards (e.g., CMS, MIPS, HEDIS).
  • Manage Data & Metrics: Partner with IT/Data Analytics to track and report program performance using KPI dashboards and ensure accurate data documentation.
  • Build Partnerships: Collaborate with providers, payers, and health systems to strengthen alliances and demonstrate program value.
  • Foster Culture: Promote a culture of accountability, teamwork, and continuous improvement.

Qualifications

  • Bachelor of Science in Nursing (BSN) is required; Master of Science in Nursing (MSN) is preferred.
  • Must hold a current, valid RN license (or temporary license) through the Arizona State Board of Nursing, or hold a compact RN license, and maintain valid licensure.
  • CPR certification is required.
  • Minimum of 5 years of nursing experience.
  • At least 2 years in chronic disease management or case management.
  • At least 2 years in a supervisory or leadership role.

Pay Range

USD $85,000.00 - USD $95,000.00 /Yr.
