Role Summary:
The Medical Coder is responsible for reviewing documentation and selecting the appropriate diagnoses and ICD-10 coding based on CMS and coding guidelines. Consults with clinicians to confirm accuracy of coding, as appropriate.
• Review physician orders, H&P’s, and Progress Notes for accuracy and compliance components. Fix and update medical orders for those errors within scope of practice. Additional errors returned to Clinical and Quality staff with recommendations for correction for OASIS accuracy and coding.
• Provide technical guidance to visiting staff and clinical management in creating medical orders and maneuvering through the electronic records system.
• Responsible to meet Agency, State and Federal deadlines for obtaining physician signature on medical orders.
• Follow up and insure the return of delinquent F2F Orders. Identify problems related to such orders processing, including insurance and regulatory requirements and follow up to insure timely processing.
• Triage Medical Order phone calls and email requests. Perform Medical Order functions including: filing, computer input, printing, and mail processing.
• Other duties as assigned by the Director of Clinical Services/Administrator.
• Minimum of 2 years’ experience in Home Care.
