The Registered Nurse – Behavioral Health is responsible for professional home nursing services to the adult or geriatric patients experiencing psychiatric mental health disorders. Provision of care includes: Mental health patient assessment, patient and caregiver education, development and revision of the plan of care, monitoring effectiveness of psychotropic medications and working with physician on dosage adjustments, psychosocial support, observation and reporting of any behavioral changes and works with patients on coping skills and independence.

Responsibilities

  • Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
  • Initiates the plan of care and necessary revisions. Provides those services requiring substantial specialized nursing skills.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  • Informs personnel of changes in the condition and needs of the patient.
  • Counsels the patient and family/significant others in meeting nursing and related needs.
  • Processes orders and notifies physician of patient needs and changes in condition.
  • Completes certification/recertification orders and discharge summaries.
  • Determines the amount and type of nursing needed by each individual patient.
  • Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
  • Supervises and teaches other nursing personnel.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
  • Appropriately utilizes ICD codes.
  • Works in collaboration with MD or Psychiatrist to monitor, request and adjust psychiatric medications.
  • Educates patients/caregivers on side effects, diet and safety factors of new medications.
  • Provides crisis services, education and individual therapy for patients, families and caregivers.
  • Identifies problems, assesses level of function and plans intervention strategies, possible placement, provides treatment and coordinates services with other members of the health care team.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Currently licensed in the state(s) in which practicing.
  • Valid driver's license with maintenance of safe driving record and an automobile that is insured in accordance with state requirements.
  • Two (2) years nursing experience in direct patient care; Home Health experience preferred.
  • One (1) year experience within the previous five (5) years working with patients experiencing psychiatric mental health disorders in an acute psychiatric or behavioral health environment.
  • Experience in gerontology and home care preferred.
