The Care Coordinator coordinates, monitors, and updates all services and service plans relating to resident care needs through service plans.

Responsibilities

•Collects health information and reviews biometric data to identify gaps in care and provides disease-specific education and interventions specific to gaps identified under the guidance and direction of provider, qualified healthcare professional, and/ or registered nurse.
•Interpret policies and procedures to personnel, patients, visitors, and government agencies as required.
• Operates as the point of contact for guest volume trends related to associate scheduling
adherence. In collaboration with the Director of Nursing, the Scheduler is responsible for criticalreal-time decisions regarding increases and reductions and re-allocation of labor.
• Tracks labor, including but not limited to: overtime and additional hours, time off requests andapprovals, attendance tracking/reporting, schedule creation and schedule changes.
• Uses creative and flexible scheduling techniques to ensure that the schedule is current and up todate at all times.
• Supports and participates in the hiring process of nursing personnel.
• Collaborates with the Director of Nursing to coordinate the assignment of staff to perform nursing care of all types, while ensuring that the staff to guest ratio meets mandated staffing guidelines fornursing care facilities.
• Coordinates successfully with other Care Coordinator and is availalbe to cover shifts as needed 7 days per week.
•Staffing phone bonus $35/day on days repsonsible. Shares responsibility with other Care Coordinator.  
 
 
 
 

Qualifications

•High school diploma or GED
•CPR certified
•CNA/HCA certified
•LPN Preferred
•One (1) year experience in a long-term facility preferred
