The Intake Coordinator is responsible for completing the referral and initial admission process.

Responsibilities

•Develop working relationships with internal sales team and facility staff to create excellent admission outcomes.
•Handles phone inquiries, checks for referrals, and processes them expediently and accordingly.
•Communicates with the Liaison and referral source, notifies them it has been received and an answer will be provided promptly on acceptance or denial of the referral.
•Reviews health records prior to accepting referral.
•Creates new patient digital charts.
•Verify primary insurance coverage and obtains authorization by calling insurance provider.
•Costs out medications and obtains approval from the Administrator, once approved notifies Liaison to complete referral evaluation.

Qualifications

•Minimum of one (1) year experience with admissions in a Skilled Nursing Facility, hospital discharge, or case management required.
High School graduate/ GED required.
•Type 40 words per minute.
•Knowledgeable of medical practices, terminology and procedures, as well as laws, regulations and guidelines that pertain to home health.
•Able to relate positively, favorably and cooperatively with others, including physicians, associates, and personnel of outside agencies and organizations.
•Must be capable of working under pressure in a busy setting, projecting a friendly and professional image. Must be versatile and adapt to changing job duties and priorities, and/or emergent or stressful situations.
