The Pharmacist is responsible for clinical evaluations of disease state, diagnosis, interactions (drug/drug, drug/disease state, etc.) for new and existing patients, including those in collaborative practice agreement protocols.

Responsibilities

•Drug information supplied to staff and/or patients and/or families.
•Responsible for contract administration and adherence for medication, contracted services, etc.
•Responsible for quality control to include capturing error report data for pharmacy and nursing as it relates to medication use.

Qualifications

•Current licensure by the applicable state as a Registered Pharmacist
•Five years of either Acute or Clinical care practice as a pharmacist to include clinical pharmacy experience in disease state management; anticoagulation medication management; IV medication compounding; Aminoglycoside/Vancomycin dose management.
•Excellent time management and verbal and written communication skills.
•CPR
