The Enrollment Specialist is the first impression patients have of Alanté Health’s mission and 24/7 CCM service. Working in a fast-paced environment, the ES will be physically located within a physician’s office or clinic that participates in Alante’ Health’s Chronic Care Management (CCM) program. Their primary responsibility is to facilitate the enrollment process of Medicare 65+ patients into Alante’s CCM program.

Responsibilities

  •  Utilize software systems to Identifying eligible patients and to verify patients’ eligibility. software
    and or make manual call to Insurance company to verify patients’ eligibility.
  • Scrutinize patient records and potentially outreach through other channels, to identify eligible
    patients meeting specific criteria for enrollment into the CCM program. This includes patients with 2 or more chronic conditions that require ongoing monitoring and management.

  • Manage the enrollment process, ensuring all necessary forms and procedures are completed
    accurately. This includes obtaining patient consent, verifying insurance eligibility, and ensuring the
    patient fully understands what enrollment entails.

Qualifications

High School diploma or GED equivalent.
A minimum of six (6) months’ experience in office administration is required.
Administrative work experience in a healthcare environment is preferred.
