The Registered Nurse plans, organizes and directs hospice care and is experienced in nursing with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Demonstrates the core nursing competencies established by the agency to ensure effective and efficient care on an ongoing basis.

Responsibilities

• Demonstrates the core nursing competencies established by the Agency to ensure effective and efficient care on an ongoing basis.

• Completes an initial and ongoing comprehensive assessment of patient and family to determine hospice needs.

• Initiates the plan of care and necessary revisions.

• Provides those services requiring substantial specialized nursing skills.

• Assists the patient with ADL’s and facilitates the patient's efforts towards self sufficiency and optimal comfort care.

• Prepares clinical and progress notes for each patient visit and summaries of care conferences

on his/her patients in a timely manner as per Agency policy.

Qualifications

  •  Graduate of an accredited School of Nursing and licensed in the state(s) in which practicing
  • Two (2) years nursing experience with recent acute care in an institutional setting, and/or recent home care experience
  • Valid driver’s license with maintenance of safe driving record and vehicle that is insured in accordance with state requirements.
